About the Role: My client, a growing NDIS Registered service provider, is seeking a dedicated RN Care Manager. You will ensure smooth operations and high-quality care for our participants.
Key Responsibilities
Conduct client assessments and develop care plans.
Coordinate medical appointments and community resources.
Advocate for clients' rights and quality care.
Communicate effectively with clients, families, and healthcare professionals.
Mentor and coach junior nursing staff.
Skills & Experience
Registered Nurse with current AHPRA Registration.
Experience in care coordination or case management.
Strong people management and organisational skills.
Excellent communication and interpersonal skills.
Knowledge of healthcare regulations and standards.
Proficiency in MS Office and CRM systems.
Current NDIS Worker Screening and Working with Children Check or willingness to obtain.

Qualifications
Registered Nurse Qualification with current AHPRA Registration
Proven experience as a care leader within the health, community or disability industry
People management skills including performance management, delegation, follow-up and goal setting with direct reports
Previous experience in care coordination, case management, or a related role, preferably in a healthcare or social services setting.
Excellent interpersonal and communication skills, with the ability to build rapport and collaborate effectively with diverse individuals and teams.
Strong organisational skills, attention to detail, and the ability to manage multiple tasks and priorities.
Empathetic, patient, and compassionate approach when working with clients and their families.
Knowledge of relevant regulations and standards related to healthcare and social services.
Proficiency in MS Office (MS Word, Excel and PowerPoint)
Previous use of CRM systems
Excellent time management skills and the ability to prioritize work
Current NDIS Worker Screening and Working with Children or willing to obtain (essential)
Knowledge of workplace health and safety legislation requirements
