Purpose of the Enhanced Primary Care Pathways and Integrated Care (EPPIC) Team

The EPPiC Team is responsible for providing community-based primary care and clinical support across the Halton region supporting people who are unattached to primary care, unhoused/unsheltered and who experience complex health care concerns including comorbid mental health and substance use concerns, including Hep C, HIV/AIDS etc. The team will work primarily in the community meeting people where they are at, including shelters, supportive housing, street level outreach and key community partner locations. This team will include Nurse Practitioners, Registered Nurses, Social Workers, Peer Support Workers and an Occupational Therapist.

Physician Responsibilities

  • Provide comprehensive primary care considering all risk factors (environmental, social, spiritual, psychological, and physical) and relevant information such as ethnic affiliation and socio-economic needs.
  • Manage clients with mental health and addiction needs, including prescribing Opioid Agonist Treatment (OAT) and anti-craving medications.
  • Empower clients to make informed health decisions by optimizing their understanding of available choices.
  • Act as a resource to patients' primary care providers, especially when there are difficulties in accessing primary care.
  • Perform clinical duties including health assessments, prescribing treatment, providing on-call service, consulting with Nurse Practitioners, being available for critical lab results, and making home visits as necessary.
  • Collaborate with and refer clients to EPPIC staff and external resources as appropriate.
  • Coordinate care with other health professionals for in-home health care patients.
  • Obtain and maintain privileges at the nearest hospital for lab and diagnostic procedures.
  • Provide information on relevant primary care resources to EPPIC staff, clients, and community residents.
  • Ensure comprehensive treatment by maintaining accurate medical records, participating in chart reviews, and discussing problem cases.
  • Work in collaboration with Nurse Practitioners and provide backup support at appropriate sites.
  • Conduct telemedicine and telehealth appointments according to policy.
  • Maintain confidentiality in accordance with Board and Ontario College of Physicians and Surgeons policies.
  • Adhere to all risk management principles and procedures.
  • Prescribe controlled medical substances per the guidelines of the College of Physicians and Surgeons of Ontario (CPSO).

Working conditions

  • The EPPIC Team operates in the community 7 days a week. The physician will work with the team to determine the most effective hours to meet the population’s needs.
  • Starting salary is $150,000 annually for part-time (0.25 FTE – 0.5 FTE).

Job Types: Part-time, Permanent

Pay: From $150,000.00 per year

Expected hours: 9.38 – 18.75 per week
