Job Duties:
- Conduct comprehensive patient assessments in the home, monitoring vital signs and documenting findings.
- Administer medications and treatments as prescribed, including wound care and IV therapy.
- Develop and implement individualized care plans in collaboration with healthcare teams.
- Educate patients and families on disease management, medication administration, and self-care techniques.
- Maintain accurate and timely documentation of patient care in electronic health record (EHR) systems.
Job Requirements:
- 2-year recent (within the last 3 years) full-time
- LPN Licensure of Hawaii
- BLS from AHA
