City/State Norfolk, VA Overview Work Shift First (Days) (United States of America) Job Summary Optima Health is a hiring a Care Coordinator for the Clinical Government Health Center n the Central Virginia Region (see below.) This position has a remote office with 50% of the time being spent in the field making face to face visits. The Care Coordinator performs a variety of casework duties and provides case management services to patients, families, and designated caregivers. Must develop, participate and monitor multidisciplinary collaboration of services to patients where appropriate. Assist adult patients and their families with personal and environmental difficulties associated with medical condition up to and including at time of terminal illnesses. Schools that are not listed on the database do not meet the standard as accredited. Degrees that have been obtained from schools outside the United States will be reviewed individually.
