We are looking to add additional full-time Registered Nurses to our dynamic team of clinicians!

We deliver professional and loving care to our patients in their homes. Our Registered Nurses must possess strong clinical knowledge in various geriatric disease processes and be proficient in performing head-to-toe assessments, wound care, blood draws, administering IV therapies, maintenance of PICC line, Mid line, Central lines, to name a few.
What Will You Be Doing?
- Make initial nursing evaluation visit to communicate with physician and determine a plan of care.
- Establish a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, and families.
- Collaborate with patients and their families, as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience
- Develop an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously review and revise the plan of care as patient condition warrants.
- Communicate timely changes in patient’s assessment/ongoing reassessment to providers that warrant consideration and/or a change in the plan of care and maintain and coordinate communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery.
Requirements:
- Licensed IN Registered Nurse (RN) is required
- Home Health or Hospice nursing experience is preferred not required
- Must be able to travel the assigned territory
- Able to participate in the on call rotation
- CPR - American Heart Association or the American Red Cross
- Electronic documentation experience is required
- Must be able to complete a minimum of three day training in the Carmel office
- Must have an active driver license and reliable transportation to drive throughout the day / territory
