We are seeking an enthusiastic and compassionate bilingual (English/Creole) Care Coordinator to join our 211 Help Me Grow team.
Responsibilities:
The Care Coordinator will respond to calls from parents, childcare providers, early educators, health professionals, and social service agencies about concerns related to child learning, developmental, emotional and/or behavioral issues with a special emphasis on those who speak Creole.
As a Care Coordinator at 211 Palm Beach, you can expect a dynamic role where each day brings a variety of tasks. Your responsibilities will include conducting assessments, providing referrals to community resources, and offering emotional support to individuals in need. With a day shift schedule from Monday to Friday, you'll have the chance to make a difference during traditional work hours. Using HMG approved screening tools, such as the ASQ and other evaluation tools, the Care Coordinator conducts screenings to determine concerns related to child learning, developmental, emotional and/or behavioral issues.
Qualifications and Experience:
To excel as a Care Coordinator at 211 Palm Beach Treasure Coast, you'll need exceptional organizational skills to manage multiple tasks efficiently and effectively. Strong communication skills are essential for interacting with individuals seeking assistance and collaborating with colleagues. Additionally, empathy and the ability to actively listen are crucial for understanding and addressing the needs of clients. Familiarity with Microsoft Office suite and database management is preferred, as you'll be required to document client interactions and maintain accurate records.

211 Palm Beach Treasure Coast is a private nonprofit 501(c)3 agency that was started in 1971 and quickly expanded into crisis counseling and suicide prevention. The agency cultivates a caring and dynamic team to service Palm Beach County and the Treasure Coast (five counties). Additional partners provided funding to support a quality, centralized access point for health, and human services information and crisis services. 2-1-1 was designated nationally by the FCC for information and referral purposes, and it became the telephone number for the agency's helpline.
Schedule:
Full time day shift Monday through Friday. Occasional weekend events.
Salary:
$20.00 - $21.48 per hour
