Case Manager Full Time Day Shift The Long Term Care Case Manager will provide a wide range of social work services to clients on the Choices for Care program, with the ultimate goal of supporting clients to remain independent in the setting of their choice. Other responsibilities: Provide case management services for assigned case load under the Choices for Care program Work in partnership with clients and their families to identify strengths, current and future goals, and unmet needs Conduct annual assessments to determine care needs, provide education and options counseling within the Choices For Care Program, assist clients with completion of Long Term Care Medicaid application and subsequent reviews, provide information about community resources and making referrals as needed, monitor services through monthly home visits, and any other tasks associated with the coordination of long term care services Provide on-going support related to adjusting to illness/disabilities, adherence to medical regimes, strengthening family support system, and coping with increased functional limitations Provide assistance to clients who may be experiencing a number of psychosocial issues including unsafe home environment, inadequate food and finances, mental illness, suicidal thoughts, increased risk of abuse, exploitation, neglect or self-neglect Education: Bachelor’s degree in Social Work preferred, but related field of study or relevant experience considered This position does not provide clinical supervision for purposes of licensure.
