JOB DESCRIPTION
POSITION: Direct Support Professional (DSP)
REPORTS TO: QIDP/IMC/Program Manager/Director
WORK SCHEDULE: Flexible, as requested by the family (not to exceed 8hrs a day)
STATUS: __X__Full-time ____Part-time ____Consultant/Contract
SCOPE OF POSITION: 360 Support, Inc, (3SI) DSP shall comply with engaging and assisting the person in the acquisition, retention and improvement of skills related to activities of daily living, such as personal grooming, household chores, eating and food preparation, and other social adaptive skills necessary to enable the person to reside in the community and also in the implementation and documentation of these skills acquisition.
Prospective employee for this position will participate in, engaging the person, implementing and documenting the programs as required.
ESSENTIAL JOB FUNCTIONS & RESPONSIBILITIES:
· Training and support in activities of daily living and independent living skills
· Support to enhance opportunities for meaningful adult activities and skills acquisition that support community integration and a person’s independence, including management of financial and personal affairs and awareness of health and safety precaution;
· Support to enhance opportunities for community exploration aimed at discovery of new and emerging interests and preferences, including activities aimed at supporting the person to have one or more new relationships;
· Support to build community membership;
· Training on, and assistance in the monitoring of health, nutrition, and physical wellness;
· Implementation of home therapy program under the direction of a licensed clinician;
· Training and support to coordinate or manage tasks outlined in the Health Care Management Plan, if applicable;
· Assistance in performing personal care, household, and homemaking tasks that are specific to the needs of the person, except that this may not comprise the entirety of the service;
· Assistance with developing the skills necessary to reduce or eliminate behavioral episodes by implementing b Behavior Support Plan, (BSP) or positive strategies;
· Opportunities for the person to seek employment and vocational supports to work in the community in a competitive and integrated setting;
· Assistance with the acquisition of new skills or maintenance of existing skills based on individualized preferences and goals identified in the In-Home Supports Plan, ISP, and Plan of Care; and
· Coordinating transportation to participate in community events consistent with this service.
· Other duties as assigned
COMMUNICATION:
· Ability to effectively communicate orally and in writing with internal and external business professionals
· Communicates with the person, family, support staff and other external bodies appropriately
DECISION MAKING:
· Make safe and sound decisions
· Must be responsible, reliable, and able to carry out job functions
WORKING ENVIRONMENT:
· The work is performed at the residence/community setting of the person supported
· The DSP is expected to be professional, kind, gentle, warm, and to develop rapport with the person.
QUALIFICATIONS:
· Must have a high school diploma and must meet all requirements for employment.
· Must have experience in the field of Intellectual Disabilities.
