The QA Coordinator will be responsible for facilitating compliance with the Agency’s Quality Improvement Program to maintain compliance with Agency standards, regulatory bodies, and standards of practice. This includes monitoring factors which directly and indirectly influence the care provided to the agency’s clients. Provide OASIS review / Plan of Care review, chart audit review, and typical review and monitoring to maintain Agency Quality Improvement measures.
Requirements:
- RN with current license to practice in states of operation. Baccalaureate degree in nursing preferred.
- Minimum two years of clinical nursing experience; home care experience required
- Supervisory experience preferred.
- Minimum of two years’ experience in a Medicare certified home health agency providing skilled nursing services including use of OASIS and electronic health record
- Demonstrates knowledge of Performance Improvement and Quality Assurance programs
- Knowledge of the regulatory requirements for home care, including the OASIS data collection and reporting requirements and OBQI expectations for Medicare Certified Agencies
- Ability to work with minimal supervision as well as collaborating within a team environment
- Has a working knowledge of home health federal and state licensing and payment regulations (i.e., Medicare and Medical Assistance), and policies and procedures
- Knowledgeable about all of Accra services
- Effective written and oral communication, interpersonal, critical thinking, conflict resolution skills, organizational, and time management skills
- Strong clinical competencies in nursing assessment, person-centered care planning and nursing interventions, medication management, and documentation
- Current driver’s license, safe driving record and reliable transportation
Essential Functions:
- Provide OASIS and Care Plan review for accuracy and timeliness
- Works collaboratively with Director of Nursing/Clinical Managers to assure documentation is complete, accurate, reflects care delivery, standards of practice and legal requirements
- Conducts record reviews, prepares written summary of findings and recommendations. Assists in the implementation of recommendations and plans for evaluating results
- Regularly reviews samples of admissions to verify; timeliness of response to referral, services provided, compliance with orders, appropriateness of amount and level of care provided
- Provides effective monitoring and oversight of home care staff with specific responsibilities with hiring, orientation and training, supervision, and evaluation of Direct Support staff
- Makes assignments based upon client needs and staff ability
- Identifies the educational needs of team members and provides or recommends educational resources and activities
- Performs or delegates in-home supervision of team members
- Monitors or delegates routinely the evaluation of staff productivity
- Conducts or arranges for Direct Support Staff competency tests
- Provides education and mentoring to staff as required for achieving quality clinical, regulatory, and business outcomes.
- Assists in ongoing, staff development strategies and activities
- Participates in all mandatory education per agency guidelines
- Identifies need for staff growth and development
- Works with staff schedulers to identify service delivery staffing needs
- Supervises and guides the delivery of quality home care services with responsibility and
- accountability to the Minnesota Nurse Practice Act, licensing standards for rehabilitation therapies; licensing, certification, and payer requirements for home care; and agency policies and procedures
- Provides skilled nursing services in accordance with the Accra Job Description for Registered Nurse and with respect to individual client needs and preferences
- Demonstrates thorough knowledge and application of a comprehensive base of nursing theory and nursing process. Initiates referrals as needed to other health team members
- Demonstrates skill in effective communication that is in accordance with the agency requirements such as:
- Care coordination
- Timely reporting of significant changes in client status to the physician,
- other members of the team, and family and/or individual representative
- Maintenance of client records with all required practice and regulatory elements
- Assuring the development, implementation, and updates of the individualized
- patient plan of care is met
- Participate in staff performance issues
- Independently, and in conjunction with the RN Director of Nursing and quality improvement staff, initiates actions to improve the quality of and client satisfaction with services
- Consistently practices confidentiality and protection of staff and client private information
- Demonstrates broad understanding and promotes safe practice for staff and clients. Responds to staff concerns about unsafe situations per established Agency policies and procedures
- Serves as a mandated reporter under the Minnesota Vulnerable Adult Act (VAA) and Minnesota Maltreatment of Minors Act (MOMA)
- Performs other duties as assigned by the Director of Nursing, Clinical Manager, Home Health Program Director.
