FUNCTION:
Provides Home and Community coordination services to those supported by ACHIEVA and their families. Supervises Home and Community Life Coach Team members. Utilizes experience and knowledge of home and community-based services to carry out responsibilities in the following functional areas: outcome development, implementation, and progress monitoring; adherence to Frequency and Duration; regulatory and agency compliance mandates. Seeks guidance and support from various other Home and Community Support managers/supervisors and employees as needed. Reports to the ACHIEVA Home and Community Coordinator Supervisor.
RESPONSIBILITIES:
* Actively lead Life Coach Team members in ensuring person-centered planning across the department including individual and group activities both at home and in the community. Establishment and implementation of new goals that support the achievement of a persons person-centered plan and outcomes according to the individuals wants and needs.
* Assist in recruiting for direct support professionals through the interview process and outreach events. Incorporates person-centered planning and choice by matching individuals with Direct Support Professionals. Ensures that all direct support staff are in compliance with applicable legal, contractual, or Agency standards.
* Orients and trains staff on the appropriate support requirements for the individual. Evaluates and reviews all assessment tools on the individual enrolled in the Home and Community Supports program. Ensures completion of any missing information.
* Manages and oversee employees schedules and service hours in the individual's homes. Documents and revises schedule information as necessary.
* Ensures effective communication and participation in meetings as required with individuals, families, providers and other individuals involved in the provision of supports. Educates families about their service options and, if necessary, explains to families how to secure funding from known mental retardation/mental health system resources.
* Identifies areas of concern and/or responds to problems; makes recommendations to resolve issues; and monitors implementation of recommended solutions.
* Monitors and evaluates employee performance and takes action to improve or sustain performance as needed. Provides guidance, direction, and support for assigned staff as needed. Collaborates with the Home and Community Coordinator Supervisor regarding any necessary corrective actions. Implements disciplinary actions as required in conjunction with Home and Community Supervisor.
* Remains available to assist, by telephone, in a crisis, at the discretion of the Home and Community Program.
* Prepares documents and/or reviews fiscal and other administrative records and modifies related program activities as necessary.
* Participates on various work teams as requested or appropriate.
* Performs other related duties as assigned. QUALIFICATIONS:
The equivalent of an Associate's Degree in the social science/human services field and one year of related experience. Case management and/or home care experience helpful. Proven organizational, database, word processing, and computer skills required. Ability to manage multiple responsibilities preferred. Good communication and interpersonal skill necessary. Valid Pennsylvania drivers license and properly insured vehicle required. Act 33/34 clearances, FBI required. Travel to multiple program offices required. Occasional late afternoons, evenings and weekends required.
