MDS Case Manager

Our Skilled Nursing Facility is seeking a motivated MDS assistant to join our dedicated team. This position is full-time, Monday through Friday.

Responsibilities

• Assist with the coordination of development and completion of the RAI process in accordance with current rules, regulations, and guidelines that govern the resident assessment in accordance with Medicare, Medicaid, OBRA, and other payer programs
• Attend and participate in continuing education programs designed to help you keep abreast of changes in your profession, as well as to maintain your license on a current status
• Ensure assessments accurately reflect the physical, mental and psychosocial status each resident
• Ensure assessments reflect optimal reimbursement for services provided
• Ensure appropriate documentation to support services provided
• Assist as needed in maintaining the Medicare PPS and OBRA assessment schedules to ensure timely completion of all assessments
• Complete entry trackers, discharge assessments. OBRA and PPS assessments are accurate and timely
• Ensure that an Initial Admission Assessment is completed within fourteen (14) days of the resident's
• Prepare for State and Federal agency audits related to RAI process and reimbursement
• Assist in establishing and managing patient care plans to address clinical needs.
• Manage MLTC insurance auth process fro short term and Long term care residents.
• Implement care coordination plan in collaboration with member, providers, MLTC Insurance, case workers and other relevant parties
• Ensure compliance with company policies and procedures as applicable to area(s) of responsibility
• Handle confidential information and materials appropriately and maintains a secure work area
• Other duties as assigned
