Community Living Services Program Coordinator

We are Achievement House!

We are a non-profit organization dedicated to supporting Individuals with Developmental Disabilities, by providing Vocational and Community Living Services. We have been serving Individuals with Developmental Disabilities in San Luis Obispo County since 1957, when we first introduced our Vocational Training Program. In 2007, we partnered with NCI Affiliates, and while both agencies have their own identities, both have a single focus - promoting independence and inclusion for adults with intellectual and/or physical disabilities through vocational training, education and adaptive living skills programs, advocacy, and community living services.

At Achievement House, our Community Living Services Program Coordinators are responsible for providing structure and supervision to CLS Instructors. Program Coordinators help to ensure services received meet or exceed company expectations of Individual’s served by following program designs and working in partnership with Vocational Service Coordinators. This is a supervisory position.

Essential Duties Include:

Maintain a courteous, positive, enthusiastic, well-mannered, and professional demeanor while working with Individuals Served and the Community.

Provide supervision and scheduling of Community Living Instructors.

Ensure ongoing positive communication with Individuals Served, their families, their care-providers, Funding Agencies, Schools, Staff, and the Community.

Assist Financial Services Coordinators in Community Living Services with billing to ensure billing for services provided by Community Living Instructors is accurate, turned in on time, and completed according to procedure.

Review Staff Case Management Documentation and ensure all documents are submitted in a timely manner.

Attend all Community Living Services Staff Meetings.

Oversee and participate in Client Advisory Committees.

Develop Individualized Service Plans (ISP’s) that reflect specific Individual’s training requirements.

Provide training to Staff to help instruct Individuals on goals indicated on their ISP, in order to assure acquisition, retention and improvement of adaptive skills and behaviors.

Understand and submit all paperwork as required by deadline including transportation paperwork, attendance, job coach notes, objective sheets, incident/accident reports, time studies and case management documentation.

Education / Experience:

Bachelor’s Degree in Psychology or experience in related field may be substituted for degree with approval from Executive Director. Minimum requirement of experience, three years of Case Management or work in a social services/human services environment. 

Qualifying Specifications:

Must have own vehicle.

Must maintain a current California driver’s license.

Must provide on-going proof of current state required auto insurance.

Must have an acceptable driving record as determined by criteria established by the agency’s insurance carrier.

Must clear fingerprints through the California Department of Justice and the FBI.

Must successfully complete First Aid / CPR training (Training is offered onsite)

Skills and Abilities Preferred:

The ability to have compassion, patience, good communication, resourcefulness, and the ability to make sound decisions.

The ability to work with minimal supervision.

Strong oral and written communication skills.

Reading, writing, basic math skills.

The ability to motivate others.

Flexible with work schedules and willing to help cover different sites as needed.

Bilingual preferred, but not necessary.

Physical Requirements:

Essential functions of this position require that employees regularly sit, stand, bend, twist and walk. Employees may occasionally be required to lift or move up to 50 lbs.
