Position: Registered Nurse, Assertive Community Treatment (Part-Time)
Function: Serves as a clinical staff member who is responsible for conducting psychiatric and physical health needs assessments, making appropriate referrals to community physicians and coordinating services with physicians once referred, providing management and administration of medication in conjunction with the psychiatrist, and providing health education/treatment/rehabilitation services.

Job Summary: An excellent opportunity for part-time work (16 hours per week) for an experienced professional seeking to play a pivotal role in changing the life trajectory of New Yorkers who have suffered extraordinarily in their lives from trauma, untreated behavioral health conditions, discrimination, and extreme poverty.
The multi-disciplinary Assertive Community Treatment (ACT) team will consist of a Team Leader, Licensed Clinicians, Registered Nurses, a Psychiatrist, Substance Abuse Specialist, Employment Specialist, Housing Specialist, Family Specialist and a Peer Specialist. The team will support program participants in the community to fulfill their potential to lead stable, happy and healthy lives. Through the application of evidence-based models of care, the team will aid program participants in making progress toward clinical stability, stable housing, social connectivity, sobriety, and other goals that improve their lives.
The Registered Nurse will provide a range of treatment, rehabilitation, and support services. The Registered Nurse will be responsible for the assessment of psychiatric and physical health needs of clients assigned to the team and assist in the development of Wellness Recovery Action Plans (WRAP). In conjunction with the Psychiatrist, the Registered Nurse will provide medication administration and management. As part of the assessment process, the Registered Nurse will identify specialty care needs for each client and make appropriate referrals to community physicians to address those needs. The Registered Nurse will serve as the primary liaison with community physicians and be responsible for the coordination of services. Additionally, the Registered Nurse will provide clients with education on a variety of health and wellness topics including disease prevention, HIV/AIDS education, smoking cessation and recovery.
The role will require field-based work out of an office in the South Bronx, on-call coverage, and a willingness to work flexible hours. Tasks:
- Develops rapport with clients in order to engage them in improving their health and wellness.
- Responds to inpatient and ER admission alerts and contacts medical provider/hospitals for admission/discharge information.
- Administers standardized health and psychosocial risk screenings according to program protocols and timeframes.
- Utilizes health screenings to identify needs and interventions and develop a comprehensive care plan
- Collaborates with members of the care team to identify needs and develop a plan to help client achieve optimal health outcomes.
- Develops WRAP and Relapse Prevention Plans to help clients understand their patterns and triggers for use and identify interventions to deal with triggers and cravings and identify supports.
- Documents all interventions and attempted contacts in the EHR in accordance with program standards.
- Works in collaboration with care providers to address Gaps in Care
- Provides comprehensive transitional care following hospitalization events in accordance with ACMH Critical Time Intervention (CTI) Protocols.
- Identifies appropriate community referrals for specialty care needs and serves as a liaison with community providers to coordinate services
- Facilitates group sessions on health and wellness including disease prevention, HIV/AIDS education, smoking cessation and recovery
- Provides medication management and administers IMs and vaccines. Plants and reads PPDs and performs phlebotomy as per orders
- Coordinates medical care with the psychiatrist
- Ensures all consumers have a primary care provider and receive annual physical and dental examinations, as well as other routine preventative care screenings.
- Provides individual health counseling as needed.
- Maintains schedule for recipient prescriptions
- Transcribes orders and updates medication administration records
- Monitors vital signs and side effects of medications and reports finds to the team
- Assesses domiciled client’s living conditions by conducting home visits
- Embraces the team model by collaborating with members of the team and providing support as needed
- Supports client goals and serves as an advocate on client’s behalf
- Regularly participates in team meetings and weekly clinical conference
- Attends in-service training as requested
- Duties as assigned by supervisor
Qualifications: License and current registration to practice as a registered nurse in NYS. BSN preferred. Experience providing direct care to consumers with serious mental illness, addiction disorders and HIV/AIDS preferred. Demonstrated competency in clinical documentation. Strong written and verbal communication skills.
Salary: Part-time (16 hours per week): $53/hour.
