What are the responsibilities and job description for the LVN Case Manager position at Actalent?
Description:
They will be supporting the case managers with complex placements to SNFs, mainly but sometimes LTACH, ALFs, and Board and Care Facilities. They will also help with readmissions, work through chart reviews, and patient interviews and teaching. They will help with enrolling patients to our safety net programs.
