What are the responsibilities and job description for the Social Worker position at Actalent?
Description:
- Provides psychosocial assessments, diagnosis, and treatment to patients in their place of residence as prescribed by a physician
- Assist the patient and family develop effective coping skills in response patient's diagnosis, treatment, and/or life situation
- Communicates timely and effectively with the rest of the patients care team (doctor, PT/OT, nurses, etc)
- Synthesize data from assessment and analyzes data to make clinical judgements regarding patient management
- Provides recommendations to modify environment to optimize patient safety and reduce the likelihood of medical and health concern errors
- Identify risks of injuries or threats to life and health in the patient's environment
- With the RN Case Manager, performs transfer and discharge planning functions
- 2-5 patient visits a day depending on their schedule and patients
- Manages documentation and gets everything done in 24 hours.
- Education focused
Additional Skills & Qualifications:
- Understanding of HHA policy and applicable laws, regulations, and standards related to the geriatric population
- Maintains current knowledge of community resources. Can identify and access appropriate external and internal resources to meet the patient's, caregiver's, and/or family's needs and to facilitate optimal patient outcomes.
- Willingness to complete quarterly/annual education requirements
- Demonstrate effective time management and organizational skills
- Needs to be an effective communicator with all parties
- Need a valid drivers license and car insurance
- Current BCLS certification
- WA social worker license
- Needs to be flexible and able to manage a schedule
