Advanced Home Care is currently accepting resumes for a Full-time, Part time, LPN or RN
The qualified candidate must be highly motivated, organized, and resourceful, with excellent communication skills that fosters relationships with the Group Adult Foster Care (GAFC) program participants and AHC associates.
The GAFC Clinician has the primary responsibility for in-home supervision of direct care providers as well as the ongoing assessment, management, and coordination of participant’s care related to services provided.
The ideal candidate will be passionate, driven, dependable, and dedicated to delivering high quality care with knowledge of community services and programs. The clinician will coordinate with the interdisciplinary team including; case managers, case coordinators, caregivers, MDs, and other clinicians in order to provide the participant with continuous exceptional care.
QUALIFICATIONS
- Current nursing license in Massachusetts is required.
- Nursing Degree from an accredited institution is required.
- Previous experience in a community health setting (e.g., VNA) is preferred.
- Evidence of a satisfactory physical and TB test done within one year prior to employment.
- Professional references with regard to competency, interpersonal skills and attendance.
- COVID-19 fully vaccinated, per state mandate for health care settings
- Consent to a Criminal Offender Record Investigation check.
JOB RESPONSIBILITIES
- Accompanies the assigned Homemaker/Home Health Aide (H/HHA) on all new personal care cases to open the case and provide any needed directions to the H/HHA and participant.
- Visits all H/HHAs, while on duty with an assigned participant, as their programs requires.
- Provides wellness education to participant as needed.
- Updates each participant’s plan of care annually and/or as needed.
- Completes and submits all required documentation in a timely and designated manner.
- Coordinates with other AHC associates and agencies as appropriate.
- Functions in accordance with Agency policies, procedures and conditions outlined in Agency contracts.
- Performs other job-related duties as requested.
- Participates in the Agency's training programs as requested.
- Monitor and assess the health of the elderly/disabled/cognitively impaired participants, keeping records of any changes as needed
- Educate participant and their caregivers (CGs) on health, safety, wellness, medication management, nutrition, and maintaining independence, in order to prevent complications and hospitalizations.
- Visit participant in order to admit to services, evaluate eligibility for services needed, and orient participant and CGs to the approved plan of care.
- Develop educational sessions related to health improvement and assist with holistic health care services based upon the participant’s need.
- Adhere to and comply with AHCS agency, State and Federal regulations and guidelines in nursing and health care activities.
- Communicate regularly with Clinical Director and submit all pertaining health care documentation in timely and qualitative manner.
- This position will require travel, but mileage will be compensated.
