Summary
The Medical Social Worker restores or enhances social or psychosocial functioning of our patients by consulting with clients and their caregiver then assessing and planning referrals and interventions. MSW will act as a liaison when collaborating with other disciplines including physicians, nursing, therapies, and other community resources to ensure complete quality care of the patient to enhance the health and overall quality of life through individualized advocacy and case management. The MSW will utilize knowledge of patient's age, medical history, background, and cultural diversity in the implementation of the patient plan of care. Qualifications
- Hold a bachelors or master’s degree with an emphasis in social work from a program accredited by the Council on Social Work.
- Complies with accepted professional standards and state license/ registration requirements.
- One (1) year experience as a Social Worker in community or hospital setting preferred.
- Obtain and maintain current BLS CPR.
- Successful completion of competency and proficiency skills evaluation specific to the requirements of the position upon hire.
- License driver with well working automobile for reliability getting to patient's homes in all types of weather.
