RN Case Manager South Team

ADVANCED HOSPICE IS GROWING, and we are actively looking for an RN in the Pittsburgh area! RN Case Managers are an essential part in quality care and coordination for patients. If you have a passion for hospice and are looking for a change, this is an excellent opportunity to work with a great, unified TEAM. Responsibilities:

  • Oversee the implementation and ongoing assessment of the patient’s plan of care
  • Work closely with the IDG team (spiritual care coordinator, social worker, hospice aide and medical director) to ensure proper coordination and needs for patients and families.
  • Ensure an accurate evaluation and work to develop and implement a comprehensive care plan that addresses patient needs.
  • Communicate patient conditions and collaborate with appropriate providers to deliver care when patient needs evolve.
  • Actively participate in IDG meetings.
  • Provide supervision of aides and nurses.

Requirements

  • Education: Associate degree in Nursing (ADN) or Associate of Applied Science in Nursing (AAS) required;

Bachelor of Science in Nursing (BSN) preferred.

  • Licensure: Current license to practice as a Registered Nurse (RN) in the State of Pa.
  • Current CPR/FA certification.
  • Valid state-issued driver’s license, automobile insurance, and reliable transportation; willingness to travel.
  • One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting.

Job Type: Part-time

Pay: $65,000.00 - $75,000.00 per year
