What are the responsibilities and job description for the Caseworker position at Advantage Care Services?
The Case Worker (CW) uses best practice and technology within a community based residential setting to teach people with intellectual and developmental disabilities how to acquire the physical, intellectual, emotional and social skills needed to live as independently as possible. The CW develops goals/objectives to meet consumers needs/wants. They ensure that at least monthly Service Monitoring Logs are completed for each individual. The CW develops each person’s Implementation Plan (IP). Community integration is a key part of training, thus the CW may transport or assist the consumer with training. They may transport to medical appointments and at times may need to monitor self-administration of medication. The CW assures health community services are coordinated for each individual’s needs as requested or desired. The CW will participate in person-directed planning (PDP) and support/services consistent with the PDP. Emergency situations may require the CW to work longer than a regular shift (e.g., if a consumer needs hospitalization or if the next shifts’ staff did not it to work. The CW also ensures completion of appropriate documentation to comply with Home and Community Based Services (HCS) principles. It is the responsibility of each CW to be thoroughly knowledgeable of the HCS principles.
CW will maintain compatible relations with Community Support Staff (CSS), Residential DCS, other CW’s, Local Authority Service Coordinator (SC), and keep effective family and community relations. The CW will work effectively with clinical staff and families in order to provide quality consistent services to the consumer. The CW must have personal transportation to/from work site.
Key Responsibilities include, but are not limited to:
- Attends annual and other staffing as needed to review and revise the PDP & IPC.
- Complete Individual Plan of Care (IPC) in a timely manner.
- Develops and ensure the implementation of each consumers Implementation Plans (IPs).
- Write progress notes in an accurate and concise manner regarding each consumer enrolled into the Home Community Based Services program.
- Gathers information to develop goals/objectives that are consistent with the needs and desires of each consumer identified in the PDP.
- Notifies the LA SC to schedule meetings to review and revise the IPC on an annual and as needed basis.
- May train with individuals towards achieving Implementation Plan (IP) goals/objectives.
- Assist with staff training for each individual (as needed).
- Ensure all recommendations for follow up per PDP and IPC is completed.
- May accompany individual to and from appointments and activities as determined by supervisor.
- Assisting with tasks delegated by a Registered Nurse that must be in accordance with state law. May supervise individuals taking medication(s) unless otherwise indicated and completes appropriate documentation.
- Ensures MRRC assessments are current and completed prior to expiration.
- Keep a current listing of guardianship status.
- Builds a support network between the individual and community members.
- Coordinates with a variety of other professionals in order to achieve outcomes for each consumer.
- Accurately completes all required Advantage Care Services (ACS) documentation and maintains current on all paperwork.
- Provides both monthly and as needed notes regarding each consumer.
- Ensure each consumer receives a face-to-face visit at least quarterly.
- Monitors utilization of services.
- Non-Essential Functions:

- Maintains a valid TX driver’s license and meets ACS insurance criteria.

- Minimum Qualifications (Work Experience/Education)Bachelor’s Degree with major specialization in social, behavioral or human services related fields; OR have a high school diploma/GED with related volunteer experience comparable to 2 years full-time work in social, behavioral, or human services or related fields; OR have a high school diploma/GED with a minimum of 2 years full-time work experience in social, behavioral, human services or related work.
- 2 years experience preferably in a residential, habilitative training program or related human service setting.
- Must have good driving record and eligible for insurance coverage by ACS.
- Must be able to pass background checks.

- Complete CPR successfully within 30 days of employment
- Working Conditions: Lifting and carrying up to 70 lbs., repeated bending, reaching above shoulder, simple and simultaneous grasping, walking, standing, sitting, twisting, kneeling, pushing, stooping, climbing stairs, operating office equipment, operating a motor vehicle, ability to see, hearing with or without aid, ability to write, count, read and speak in English, ability to reason are all characteristics of the position. There will be work outside and inside facilities, travel by personal car and exposed to changing weather conditions. Occasionally, you may travel to sites outside the city area. There will be tasks that require working closely with others as well as working alone. The hours are variable according to client needs; however, hours are generally 8-hour shifts. The CW may occasionally transport consumers in own vehicle; transportation of consumers must be provided in accordance with applicable state laws. The CW may assist with wheelchair transfer of non-ambulatory participants. The CW may be exposed to various medical conditions and communicable diseases. Work will generally take place in the community and in the consumer’s home. The CW will maintain a flexible schedule as determined by the supervisor.
- REASONING ABILITY: Must complete initial, annual and periodic training, as needed.
