Assists in maintaining effective communication between the patient, caregiver, physician, contact person, payer source and the rehabilitation hospital. Fosters positive relationships. Adheres to the Florida Hospital Corporate Compliance Plan and to all rules and regulations of all applicable local, state and federal agencies, and accrediting bodies The value you’ll bring to the team: Utilizes established professional relationships and methods of communications with referring and supporting health care individuals, organizations and payer source to ensure clear and concise exchange of necessary information in a timely manner. Effectively evaluates and assesses patient candidacy for inpatient rehabilitation. Formulates and issues decisions regarding acceptance or denial of candidates in accordance with established admission criteria. Recommends continuum of care alternatives for those denied admissions. Facilitates patient’s movement through the program by providing proactive planning and creating solutions to facilitate seamless care. Advocates for patients’ ongoing and discharge needs. Supports quality standards and initiatives set by the business unit. Willingly adjusts and adapts to changes in workload by altering working hours to assist in other areas as approved and observed by supervisor. Practices principles of employee and patient safety when performing daily job activities via observation. Perform follow up call backs at 7 and 90 days after discharge Qualifications The expertise and experiences you'll need to succeed: EDUCATION AND EXPERIENCE REQUIRED : Three years of experience in respective clinical field EDUCATION AND EXPERIENCE PREFERRED : Experience in a rehabilitation setting LICENSURE, CERTIFICATION, OR REGISTRATION REQUIRED : Current valid State of Florida or multi state license as a Registered Nurse OR Current active Florida license as PT, OT or ST Basic Life Support (BLS) Certification This facility is an equal opportunity employer and complies with federal, state and local anti-discrimination laws, regulations and ordinances.
