The role you’ll contribute:
The Hospice Social Worker delivers various social work services to hospice patients and families. The purpose of social work intervention is to provide initial psychosocial evaluation, ongoing psychosocial support, direct casework services, and referral to community resources to assist with end of life care needs, as well as to engage in community education and outreach. Through collaboration with the hospice interdisciplinary team, the Hospice Social Worker implements, evaluates, and changes as needed individualized plans of care. He/she assists in identifying and implementing measures and interventions to assist patients and families in reaching desired outcomes. The Hospice Social Worker reports to the Patient Care Manager.

Demonstrates critical thinking skills, analysis and sound clinical judgment in the care of hospice patients/families and uses the social work process to address the physical, emotional, psychosocial, and spiritual needs of patients and families
Conducts a systematic assessment of the physical, psychosocial, and functional aspects of the patient and family and their impact on potential outcomes
Determines patient/family problems, issues and needs to provide an individualized plan of care with the collaboration of the interdisciplinary group
Participates as a member of the Bereavement Team as assigned
Maintains accurate, timely documentation and meets Hospice Medicare Conditions of Participation requirements
Is proficient with electronic medical record documentation
Qualifications
The expertise and experiences you’ll need to succeed:
Graduate of a master’s program in Social Work required
Must have one year experience in a healthcare setting
Must be licensed or eligible for licensure within the State of Florida (LCSW)
Must maintain active driver's license and auto insurance
