AdvisaCare Home Health is currently seeking a PRN Home Health Physical Therapist to join our team! As a Home Health Physical Therapist, you will have the opportunity to provide individualized care to patients in the comfort of their own homes. - Perform evaluations, establish goals and develop treatment plans for patients based on individual needs and diagnosis
- Assess and document patients' progress towards achieving their goals
- Provide education to patients and their families about home exercise programs and proper body mechanics
- Coordinate care with other healthcare professionals involved in the patient's treatment
- Maintain accurate, thorough and timely documentation of patient care and progress
- Participate in interdisciplinary team meetings and provide input on patient care
- Collaborate with other healthcare professionals to ensure continuity of care
- Current Physical Therapy license in the state of Michigan
- Minimum of one year of experience as a Physical Therapist with home health care experience is preferred, but not required
- Strong communication and interpersonal skills
- Ability to work independently and manage time effectively
- Valid driver's license and reliable vehicle
