Major Responsibilities:
- Provides excellent patient care using independent judgment and clinical expertise while following state and Clinic regulations.
- 1)Selects appropriate assessment procedures to evaluate the abilities and limitations of patients of all ages and diagnoses and plans appropriate treatment programs using department protocols when applicable, based on physician referral, evaluation results, patient/family goals and interests, sound clinical judgment and other pertinent available information. May received referrals from physicians or peers to evaluate and treat more challenging cases.
- 2)Will develop a plan of care, discuss with the patient and/or responsible party, and obtain verbal (or written) informed consent. Periodically re-evaluates patients utilizing clinical judgment for progression/modification
- 3)Administers daily treatment, ensuring that the treatment plan is age appropriate and patient specific. Makes recommendations to physicians regarding plan of care, which may include change of treatment, change of frequency, additional referrals, orthosis, or durable medical equipment.
- 4)Indirectly supervises physical therapy assistants and directly supervises physical therapy extenders in administering treatments to patients and will teach them appropriate techniques to perform with specific patients.
- 5)Provides education to the patient, family, or significant others based on patient needs. Identifies patient learning readiness, preferences, and learning barriers and will address those patient specific needs.
- 6)Uses review of their documentation as a tool to develop assessment and treatment skills.
- 7)Instructs patients, family, or significant other in a home exercise program and use of home equipment. Will appropriately progress the home program and activity level throughout the episode of care.
- Completes documentation of patient in the electronic medical record.
- 1)Will document all patient evaluations, plans of care, and treatments in the electronic medical record in a timely manner with feedback from a clinical mentor. Documentation will follow established templates and meet Medicare requirements.
- 2)Accurately completes coding and billing for services provided. Documentation will support all charges and will appropriately reflect the skilled services provided
- Participates in ongoing educational activities
- 1)Participates in program development and departmental initiatives to achieve Clinic and department goals
- 2)Meets all continuing education requirements needed to maintain licensure
- 3)Keeps abreast of current practices via literature, networking, internal/external, etc
- 4)Provides input on learning needs of individuals or the team and offers suggestions for implementing training and education.
- 5)Participates in orientation and integration of new associates as needed
- Responsible for maintaining productive daily operations
- 1)Responsible for maintaining productivity metrics.
- 2)Monitors openings and/or cancellations in their daily schedule and takes action to effectively fill the openings with patients
- 3)Utilizes ancillary staff efficiently and ethically based on practice act and payer provider rules
- 4)Upholds all policies and procedures of Advocate Health Care/Dreyer Medical Clinic.
Licensure:
- Physical Therapist
Education/Experience Required:
- * BA or BS (Prior to 2002) or Certificate in Physical Therapy or MPT or DPT from an accredited Physical Therapy program. New grads must provide final transcripts, 2 letters of recommendations from clinical instructors, and diploma
Knowledge, Skills & Abilities Required:
- * Basic typing and PC skills * Excellent customer service skills and responsive patient care values. * Ability to communicate professionally and effectively; verbally and in writing with patients, family, associates and providers. * Analytical ability to evaluate patient condition, capabilities, and progress. * Professional knowledge to evaluate, plan, and administer appropriate treatment safely. * Good organizational and prioritizing skills with good attention to detail. * Ability to learn proper billing and coding as related to department.
- * Eligible for licensure in Illinois with proof of application for licensure submitted if out of state applicant. If new graduate - must have test date within 6 months of graduation. * BLS CPR Certification
Physicial Requirements and Working Conditions:
- * Physical ability to safely lift, transfer, position, and manually treat patients. * Ability to travel between sites. * May have excessive noise, poor temperature regulation. * Work requires standing/walking most of day coupled with intermittent lifting, positioning, transferring, or manually treating patients. * Use of EMR requires repetitive use of keyboard and mouse. * Flexible hours are possible, but hours may exceed 40 hours per week. * May need schedule flexibility to accommodate patient access. Possible early morning or late evening rotations. * Possible exposure to chemicals or blood-borne pathogens.
- Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
