What are the responsibilities and job description for the Patient Care Coordinator position at Agrace?
Overview:
The Patient Care Coordinator is responsible for scheduling Homecare staff and providers. They also assign newly
admitted patients to care groups according to established staff matrices.
Responsibilities:
Scheduling (Percentage of time: 70%)
a. Complete all Homecare discipline scheduling (e.g. RN, SW, CNA, etc.).
b. Complete Face-to-face (FFE) scheduling.
c. Complete Provider scheduling.
d. Coordinate staffing to meet patient needs as a result of staff call-ins/absences.
e. Assign patients to teams/care groups ensuring consistency of staff, when applicable, and maintaining
established care group matrices.
2. Quality and Service (Percentage of time: 25%)
a. Ensure timely assignments of new admissions as well as accurate and timely scheduling of existing
patients requiring C.N.A., RN and/or SW support.
b. Report patient and staffing concerns to Supervisor, Patient Coordination.
a. Problem solve immediate visit scheduling needs.
b. Identify department efficiencies to aide in scheduling.
3. Other Duties (Percentage of time: 5%)
a. Participate in internal work groups, meetings and committees relative to position responsibilities.
b. Ensure compliance with Agrace policies and protocols, state, federal, and other regulations.

Qualifications:
1. High school diploma or GED or equivalent combination of education and work experience, required
