Position Summary:
This position can be either an LVN/LPN or RN licensure. This individual will direct patient care to patients as assigned.
Maintains a safe and efficient working and treatment environment per facility policies and procedures.
Communicates effectively with the treatment team to ensure safe, quality care is provided to all patients.
Responsibilities:
- Carries out all aspects of patient care and other assignments as directed
- Performs ongoing observation of the patients and milieu to ensure active treatment is provided and patients are participating.
- Reports or documents changes in patient’s condition or behavior which includes agitation, high‐risk behaviors, changes in vital signs, and eating, sleeping, and toileting patterns.
- Intervenes appropriately with patients experiencing a behavioral crisis situation.
- Initiates and/or participates in provision of emergency medical care when indicated.
- Participates with the treatment team in implementation of the individualized plan of care.
- Provides patient educational information for identified learning needs.
- Facilitates educational/activity groups as assigned or when indicated using approved materials.
- Ensures compliance with infection control protocols and/or policies by patients, visitors, and staff.
- Performs environmental and safety rounds and complete environmental check sheet as assigned. Takes action to mitigate risk and report identified problems to the Program Director, Director of Nursing or Designee.
- Complies with National Patient Safety Goals and provide immediate feedback if noncompliance is identified.
- Ensures a First Dose Review has been completed prior to the administration of any new medication.
- Obtains clarification on medication orders that are unclear, do not include all the required elements or include Do Not Use abbreviations.
- Reinforces medication teaching on all new medications and respond to additional questions and concerns the patient may have regarding medications.
- Documents medication variances and/or adverse drug reactions on the appropriate form and notify the patient, practitioner and facility leadership as directed per policy.
- Controls access to the medication rooms and/or medication dispensing machines in accordance with facility policy.
- Ensures medication room and equipment is clean, including no personal belongings in the medication room.
Qualifications
Education, Skills, & Experience Requirements:
- High School diploma or GED; Associate’s or Bachelor’s degree preferred.
- Graduate of an accredited LPN or LVN or RN program.
- CPI, CPR / First Aid; Valid State Driver’s License if required by State Law and operating a company vehicle.
- In addition to State requirements, one year acute inpatient with psychiatric care experience preferred.
- Ability to provide excellent service to internal and external customers.
- Ability to understand and comply with government, regulatory, and company rules.
- Ability to record, report, and maintain confidentiality of information and respond to requests for information.
