What are the responsibilities and job description for the Triage Nurse position at AllaraCare Medical Group?
Middleton Family Medicine is looking for an enthusiastic, organized full-time Triage Nurse for our busy medical practice. The primary function of this role is to evaluate and assess our patients’ health care needs and determine a plan of action by utilizing established triage protocols and guidelines. This position requires a candidate with outstanding critical thinking skills, excellent verbal and written communication skills, computer and EMR proficiency, and the ability to work both independently and as part of a team.
Essential Duties and Responsibilities:
- Manage nurse-directed telephone calls, patient portal messages, and walk-in patient inquiries.
- Triage patients to assess and evaluate health questions and concerns, including screening the illness, facilitating a provider visit when necessary and giving patient self-care advice when appropriate.
- Develop an assessment based on the conversation with the patient and information within the medical record.
- Determine level of care and timeliness needs of visit i.e., emergency room, same day, this week, next week, etc.
- Schedule routine and acute appointments for patients based on their healthcare needs.
- Monitor patient flow in the clinic to ensure patients receive all recommended services in a timely manner.
- Exercise nursing skill and care to advise patients according to nationally-recognized triage protocols
- Respond with the appropriate standard of care for specific patient cases and document triage assessment, care plan and instructions, follow up notes, provider input and patient responses in the electronic medical record (Athena).
- Manage prescription refills per protocol.
- Answer and address clinical questions sent by patients via the patient portal.
- Use sound judgment in managing calls and effectively handle challenging situations both on the phone and in the office as needed.
- Review test results as they are received, collaborate with the provider if action is required, and communicate to the patient per provider instructions.
- Complete all other duties as assigned.

Minimum Requirements:
- At least one year related medical experience required
- Active RN or LPN license required.

Physical Demands and Requirements:

- Work is subject to frequent interruptions and may be subject to angry or upset patients.

Pay: $40.00 - $45.00 per hour
