Alpha Home Health and Hospice is a Medicare-certified, 5-Star rated agency that is seeking an experienced and caring full-time Hospice Nurse Practitioner (ARNP) for our Hospice and Palliative Care division in the greater Snohomish County area!

Essential Duties

• Maintain patient symptom and pain management
• Complete Face-to-Face and eligibility visits, including home health to hospice bridge program
• Create, review, and revise patient's plan of care
• Identify comfort needs & provide holistic quality of life care in coordination with attending physician
• Provide patient and family/caregiver support and education
• Practice cohesive and collaborative care with the healthcare providers and hospice staff
• Be integral to the building of a Palliative Bridge Program
• Establish, and nurture staff and community referral source relationships

Requirements and Qualifications

• Nurse Practitioner (ARNP) licensure of one year or more
• Possess current Medicare Part-B credentialing, or have the ability to do so
• Prior Hospice experience required
• Be a licensed driver with an insured automobile in good working order

Pay: $65.00 - $75.00 per hour
