Position Summary
Promote and restore the health of persons supported by completing the nursing process; collaborate with physicians and multidisciplinary team members; provide physical and psychological support to persons supported, friends, and families. Supervise assigned team members. The responsibilities and duties listed below are representative and not all inclusive.
Key Responsibilities
→Conduct ongoing assessments as determined by person’s condition and/or the facility policies, procedures, or protocols and reprioritize care accordingly.
→Document person’s assessment findings and responses to nursing intervention and progress toward problem solution.
→Encourage person supported to participate in the development and implementation of their medical plan of care.
→Develop and maintain a positive rapport with internal and external entities involved in each person’s medical plan of care.
→Educate all staff involved in providing care to the persons supported, assuring understanding of each person’s medical plan of care.
→Assist support services in developing, implementing, and conducting staff and persons supported with ongoing education and training program related to medical.
→Review prospective persons’ files and serve as a member of the Admissions Committee.
→Chair the Infection Control and Sanitation Committee.
→Ensure staff competencies regarding medication administration and compliance to Alpha Opportunities, Inc., medication policy. When necessary, review and modify this policy with Program Services Director with CEO approval.
→Monitor and review all staff who pass medication in the licensed groups homes for proper medication passing technique; conduct periodic observations.
→Maintain all medical records to facilitate compliance with Title XIX standards.
→Review, monitor, and track, as appropriate, all medical GERs.
Knowledge, Skills, and Qualifications Education:
Education and Experience:
Registered Nurse: Required: Nursing diploma or Associates. Preferred: Bachelors degree from an accredited school of nursing. At least 2 years’ experience as a RN with at least 1 year in a similar facility or skilled nursing/long-term care.
OR;
Licensed Practical Nurse: license in the state of North Dakota. At least 2 years' experience as an LPN with at least 1 year in a similar facility or skilled nursing/long-term care.
Other:
Flexible and attentive to detail
Excellent time management and organizational skills; skills in scheduling
Excellent listening, oral, and written communication skills
Basic computer skills and familiar with electronic medical records
Work independently and as part of a collaborative care team
Certification and training—provide proof of and/or training is provided: CPR Certification, First Aid Certification, any required training
Pass all prehire requirements such as criminal background and drug screen
Maintain a valid driver’s license and auto liability insurance
Minimum Physical Requirements:
Strength: Sufficient to assist with lifting and transferring a person(s) and perform CPR. Ability to lift twenty (20) lbs. Moving, lifting, or transferring of person(s) may involve lifting of up to fifty (50) lbs., as well as assist with weights of more than one hundred (100) lbs.
Mobility: Sufficient mobility to bend, stoop and bend down to the floor, ability to move around rapidly and in small, confined areas.
