Do you have a caring heart and genuine patience for individuals with developmental disabilities? If so, ACT wants you to join our team. The Community Support Professional provides assistance and companionship for individuals living in their own homes who need a little assistance each week to live their best lives. The work shift is typically shorter, and this is an ideal position for a student who wants to gain invaluable experience in a people-compassionate field. After the initial 80 hours of paid training, candidates may expect to work 2-10 hours per week, and more.

Job Summary: Community Support Professional works directly with an individual to provide the specific services to meet the individualized needs of that individual. Community Support Professional may also work with a group of individuals (ASYP or ACTion Club), in ratios between one and six to provide supports needed for individuals to participate in selected activities and progress on individual goals. Programming is varied and may occur in a variety of settings. The Community Support Professional works as a member of a team and reports to a Program Supervisor.

ESSENTIAL FUNCTIONS OF THE JOB:

  1. Accountable to the needs and desires of the individual in their home and/or community setting providing opportunities to develop skills identified in ISP program goals.
  2. Provide services to meet the needs of the individual including as needed: physical assistance, grocery shopping, cooking, cleaning, hygiene, personal care, teaching life skills and/or social skills and other basic living needs.
  3. Maintain daily notes and documentation for individuals supported. Documentation of services provided should be entered into the electronic record within 24 hours.
  4. Provide transportation if needed as part of the function of goals of the individual.
  5. Maintain positive and effective communication/rapport with individuals, co-workers, and stakeholders'including, but not limiting to: greeting individuals and guardians/staff, communicating with guardian/staff regarding services, representing ACT positively in the community, ensuring proper communication and teamwork with co-workers, keeping supervisor and/or management informed of information relevant to individuals or the program, as appropriate.
  6. Advocate for individuals with disabilities whenever possible.
  7. Ensure Health and Safety standards are followed by ACT as well as all service sites.
  8. Ensure HIPAA compliance.

ADDITIONAL JOB FUNCTIONS:

  1. Keep Supervisor informed, in a timely manner, of situations, concerns, or problems related to individual's personnel matters, or services. Bring any unusual situations to the immediate attention of the Program Supervisors.
  2. Maintain current certification in Abuse/Neglect Prevention, CPR, First Aid, Positive Behavior Support, HIPAA, and all others required by the agency.
  3. Regular attendance and punctuality.
  4. Other duties as assigned.

Pay Range for this position is $17.50 to $18.50 and will be based on experience, education and currently held certificates which are relevant to the field. $1 shift differential on all evening and a $3 differential on weekend hours.

Upon completion of six months of employment, you will receive a $.50/hour rate increase. Upon completion of 12 months of employment, you will receive a second $.50/hour rate increase.
