Licensed Practical Nurse (LPN)

Summary:
Under the supervision of a Registered Nurse (RN), the Licensed Practical Nurse (LPN) provides direct patient care to residents in a skilled nursing facility and rehabilitation center. The LPN is responsible for administering medications, monitoring patient conditions, and assisting with activities of daily living.

Essential Duties and Responsibilities:

• Provides direct patient care, including but not limited to:
  • Administering medications and treatments as ordered by physicians
  • Monitoring patient conditions and vital signs
  • Assisting with activities of daily living, such as bathing, dressing, and grooming
  • Performing wound care and other treatments as prescribed
  • Collecting and documenting patient data
• Collaborates with the interdisciplinary team to develop and implement patient care plans
• Participates in patient education and discharge planning
• Maintains accurate and up-to-date patient records
• Adheres to all facility policies and procedures, including infection control and safety standards
• Assists with resident admissions, transfers, and discharges
• Participates in quality improvement activities
• Performs other duties as assigned

Qualifications:

• Current LPN licensure in the state of Ohio
• Basic Life Support (BLS)
• Experience in long-term care or rehabilitation preferred
• Strong clinical skills and ability to prioritize tasks
• Excellent communication and interpersonal skills
• Ability to work effectively as part of a team
• Ability to lift and transfer patients

Physical Demands:

• Ability to stand, walk, and sit for extended periods of time
• Ability to lift and transfer patients
• Ability to perform tasks requiring manual dexterity
