Position Summary: Responsible for the overall identification and coordination of Addus Patients between the service lines of Personal Care, Home Health and Hospice.
Essential Duties:
- Identify Home Health and/or Personal Care patients that are at risk needing end of life care.
- Communicates with the Home Health and Personal Care teams about patients that may be eligible for hospice.
- Facilitates Goals of Care discussions with potential patients and families.
- Follows up on the Critical Incident Reports from Personal Care to identify patients that may benefit from either Home Health or Hospice.
- Communicates with providers concerning bridge patient's eligibility for hospice.
- Participates in community education, patient education, and provider education.
- Works closely with Hospice Admission Coordinators to ensure timely initiation of care.
- Assist with finding alternative services when patient is not a candidate for Ambercare.
- Abides by all regulations, policies, procedures and standards.
- Perform other duties as assigned.

Position Requirements:
- Be eighteen (18) years of age or older
- Associates Degree in marketing, business administration, or other related fields is preferred. Acceptable combination of experience and/or training may be considered in lieu of formal education.
- At least two years' experience in a homecare, hospice and/or healthcare related field preferred.
- Demonstrates good verbal and written communication, and organization skills.
- Ability to complete necessary documentation appropriately, legibly and timely.
- Strong technology skills required.
- Ability to handle stressful, delicate and confidential situations.
- Strong customer service skills and ability to keep good relationships with patients, families, co-workers and other business partners.
- Valid driver's license with an automobile that is insured in accordance with state requirements.

Job Description Summary
Responsible for managing all aspects of the patient intake process, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes.
Essential Job Functions/Responsibilities
- Reviews referrals from physicians and discharge planners for eligibility and appropriateness of hospice services.
- Ensures compliance with all state, federal, and Joint Commission referral/intake regulatory requirements.
- Provides management with recommendations on improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensures maximum third party reimbursement through direct oversight of insurance verification and authorization processes.
- As a representative of HPA, builds and monitors community and customer perceptions as a high quality provider of services.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Maintains comprehensive working knowledge of Hospice Partners of America contractual relationships and ensures that patients are admitted according to contract provisions.
- Participates in quality assessment performance improvement teams and activities.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by Hospice Partners of America
