Our hospice nurses closely collaborate with the attending physician, patients and their families, and other members of the patient care team to provide and maintain continuity of patient care to achieve excellent pain and symptom management and high quality end-of-life care for the patient. Our hospice care focuses on the whole person – combining medical care and support for emotional and spiritual needs, as well as support for loved ones and caregivers
Hospice RN Case Managers:
- Work Monday - Friday with a potential on call and Holiday coverage schedule.
- Case Manage 15-18 patients
- Attend Interdisciplinary Team Meetings
- Contribute to team huddle calls and receive updates on your patients daily
- Assign Aide Visits
- Coordination of Care with Social Workers, Chaplains, Volunteers and Bereavement team
- Develop and nurture relationships with facility staff and leaders throughout the community

Requirements:
- Current, unencumbered license to practice as a Registered Nurse specific to the state(s) the employee is assigned to work/cover for the company.
- One (1 ) year of experience as a Registered Nurse or eligible for RN intern program.
- Current CPR certification
- Valid Driver’s License and reliable transportation and agency required liability insurance to complete home visits
