Our client in Atwater, CA seeks a dedicated, passionate Physical Therapist to provide care to patients in the Atwater / Merced, CA area.
Experience as a PT is preferred but if you are in the beginning stages of your career with less than a year experience, you are still welcome to apply.
POSITION DETAILS :
- Pay : $3,318-$3,516 per week DOE.
- Location : Atwater / Merced, CA
- Assignment Length
26 weeks
- Schedule : 4x10s. Flexible schedule (Mon-Thurs, Tue-Fri, Wed-Sat)
- Transportation
Reliable transportation required. Mileage reimbursed at $0.67 per mile. Travel between homes is approximately 75-100 miles per week.
JOB DUTIES :
- Obtains all pertinent medical history from patient, family, or significant others
- Performs PT assessment visit and documents accordingly
- Performs home safety check and environmental assessment of the patient's home environment
- Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation
- Functionally reassess the patient for on-going therapy
- Determines medical necessity for other services
- Evaluates the patient's ADL and iADL abilities
- Effectively communicates ongoing with patient and family regarding the POC and progression of such
- Effectively instructs patients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc)
- Effectively communicates with other disciplines in the case
- Communication with the patient's physician (verbally and / or in writing) to obtain effective treatment modalities and / or rehabilitative therapy modalities
- Assists the physician in evaluating patients by applying diagnostic and prognostic muscle, nerve, joint and functional ability tests
- Submits ALL OASIS assessments / documentation within 48 hours of the OASIS visit
- Participates in staff meetings and in-services
- Maintains professional licensure and actively seeks out educational experiences to enhance the practice of home care physical therapy and the benefit of the patients serviced
- Other duties as assigned
REQUIREMENTS :
- Current CA Physical Therapist License
- Current BLS
- 1 year Home Health experience.
- Reliable transportation (Mileage reimbursed)
