What are the responsibilities and job description for the The Wright Center-Behavioral Health Department position at AmeriCorps?
The BH CHW will work closely with the Referral Coordinator/Social Worker, Director of Behavioral Health Integration, and other medical and BH providers to provide Behavioral Health Integration and social service resources to Wright Center Behavioral Health patients that are struggling with mental health issues, food insecurities, housing insecurities, and lack of insurance. This role will be a bridge between behavioral health and primary care medicine. The BH CHW will contact patients via telephone or in person at their appointments to provide touchpoints, check ins, and referrals to social services to close the gap in their treatment. The BH CHW will provide support and referrals to those in need of services. The BH CHW will also assist with completing BH outreaches and Integrative Care Plans.
Member Duties : The member will be responsible for working collaboratively and effectively with community members and patients to improve access to mental health screenings and services. The member will educate patients and the community on resources such as crisis centers in our area, the importance of BH screens, social service agencies, and insurance options to get the treatment that they need. The member will be responsible for assisting the Referral Coordinator/Social Worker, the Behavioral Health Director, counselors, and BH staff with outreaches at TWC. The member will also address socio economic factors preventing the patient/co9mmunity member from obtaining mental health services, medications, food, etc. This is a new integrative initiative we are looking to spearhead and as of right now we do not have the staff to assign these duties to. The member will continuously expand their knowledge bae through attending trainings and conferences on the subject matter related to this mission.
