Job Description
Anchor House is a Faith-based Intensive Residential Treatment program Licensed by OASAS (Office of Alcohol and Substance Abuse Services). The duration of our program varies from 12-18 months (or less), based on individual progress.
Our fifty bed men’s facility and twenty-eight bed women’s facility offers gender specific curricula at each site. Anchor House also provides treatment services to pregnant women on a case by case basis.
Promotes the concept of individuals taking greater responsibility for overall health and actively participate towards goal attainment.
Helps facilitate enhanced independence and self-sufficiency for behavioral and physical health relationship
Collect specimens for new residents for toxicology as specified in OASAS regulations; ensure all specimens are sent to the lab and results charted and presented at weekly interdisciplinary meetings
Provide a health assessment within 7 day of client admission. Collaborates with counselor and writes the treatment plan for client’s health.
Administer IM medication to clients. Pre-pour oral medications for individuals who are on supervised medication. Maintain Medication Log and Inventory
Observes and records response to treatment/medication and presents during weekly interdisciplinary meetings
Maintains all medical records and performs QA activities; performs accurate and complete audits of medication transported, received, administered, stored or disposed in accordance with standard regulations.
Monitor chronic conditions not limited to and including diabetes, hypertension and GERD and chart progress weekly and present at the weekly interdisciplinary meeting.
Provide a health assessment within 7 day of client admission
Make all medical appointments and referrals. Develop a system to monitor, track and report all appointments and present at weekly interdisciplinary team meetings.
Attend weekly interdisciplinary meetings for the purpose of collaboration and coordination of services
Lead Health Education workshops and conducts health screenings for identified individuals
Conduct Health Screenings
Provide in service trainings on the most current information as it relates to health, medical related addiction information/news
Attain and maintain competency in the various evidenced based practices including Wellness Self-Management and Integrated Dual Disorder Treatment
This position may require the performance of other essential and non-essential job duties in addition to those enumerated above which will be determined at the discretion of management.
Qualifications:
Current NYS RN license
Knowledge of adult psychiatric rehabilitation preferred
Experience in working with chronic populations in the context of a residential treatment and recovery model
Must have experience with chemical dependence residential and/or outpatient treatment.
Basic computer skills
