SUMMARY OF POSITION FUNCTIONS
The Intake Coordinator / Nurse Schedule Coordinator is responsible for the scheduling visitation of home health clinical services.
ESSENTIAL POSITION FUNCTIONS
- Conducts all daily patient referral and intake operations including providing direct oversight of the establishment and implementation of intake policies.
- Ensures compliance with all state, federal, and ACHC referral / intake regulatory requirements.
- Implements improved work methods and procedures to ensure patients are admitted in accordance with policy.
- Establishes and maintains positive working relationships with current and potential referral sources.
- Ensures seamless transition of patients to home care by providing direct oversight of patient education and preparation for home care, plan of care initiation, and coordination of care with multiple service providers.
- Ensures maximum third-party reimbursement through direct oversight of insurance verification and authorization processes.
- Gathers, collates, and reports referral statistics including key customer referral trends.
- Provides leadership in strategic planning including identifying opportunities for additional or improved services to meet customer needs.
- Maintains comprehensive working knowledge of the company’s contractual relationships and ensures that patients are admitted according to contract provisions.
- Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the company.
- Comply with the agency’s privacy practices and procedures related to client and employee records and all state / federal health privacy laws as outlined by HIPAA.
- Demonstrates teamwork and prompt and regular attendance to work to ensure that quality care and services are provided to the clients we serve.

SPECIAL REQUIREMENTS
- Exceptional communication skills / telephone skills.
- Attention to detail.
- If making home visits, must be a licensed driver with an automobile that is insured in accordance with state and / or organization requirements and is in good working order.
EDUCATION & QUALIFICATIONS
To perform this position successfully, an individual must be able to perform each essential duty satisfactorily. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
- High School Diploma required. College degree preferred.
- At least one (1) to three (3) years’ experience in home health care.
- Knowledge of governmental regulations, ACHC standards, and private payer practices.
- Demonstrates good communication, negotiation, and public relations skills.
- Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
