What are the responsibilities and job description for the Case Manager - Transitional Care position at AnMed?
Will coordinate transitional care followup for high risk patients related to readmission prevention efforts including telephonic case management. Provides services as consultant, educator, and clinical oversight focusing on the needs of patient populations, family, and community while interacting with multidisciplinary to influence the quality of patient care and coordination. Provides these services to patients of all age populations in both inpatient and outpatient settings.
Qualifications: Current SC RN license required. BSN required. Three (3) to Five (5) years clinical experience. Excellent communication and analytical skills required. MSN preferred.
