What are the responsibilities and job description for the Life Skills Counselor position at Apex Community Care?
Position Summary:
Provide individualized skills building and case management for clients with HIV/AIDS and a history of substance abuse in residential setting. Assess needs, evaluation of functioning and create individual plans with client participation to identify goals and interventions. The Life Skills Counselor is also available on a referral basis to work with community clients to facilitate individual and/or group skills building.
Responsibilities:
- Maintain caseload of clients who reside at Apex group residence
- Provide intensive Case Management
- Complete a comprehensive Needs Assessment for clients and identify any needs related to entitlements, medical, transportation, job skills and other areas as identified
- Make all appropriate referrals to meet needs as identified on Needs Assessment and advocate for services as needed
- Assess client's level of functioning and ability to follow through on referrals and provide assistance as needed
- Develop and monitor Individual Care Plan with client participation to identify goals and interventions
- Monitor client's follow through with ICP making revisions as needed
- Assist clients with learning life skills through instruction, coaching, role modeling or through referral for specialized education if needed (i.e. job skills training, cooking, budgeting, shopping)
- Maintain appropriate documentation in residential case files according to standards and maintain notes in charts
- Maintain ongoing communication with supervisor regarding day-to-day occurrences at group residence
- Monitor and report any medical /behavioral health needs / issues of clients at Group Residence, communicate with appropriate parties
- Participate in bi-weekly House Meeting with residents and staff
- Provide transportation with Apex Community Care van for clients when needed
- Work with residential staff to coordinate activities, recreational or otherwise, for residents
- Serve as a flexible team member of residential staff and act as generalist to provide other duties as needed or as assigned by supervisor
Qualifications:
- College degree preferred can be substituted with combination of experience and college credits
- Experience in human service field particularly in residential setting
- Knowledge of HIV/AIDS, substance abuse, case management practices
- Valid CT driver’s license
