This is NOT a fully remote position. Field visits are required.
Since this is field position; what are the location requirements? Field visits are completed in NYC (all 5 boroughs), Westchester and Long Island
What are the day to day responsibilities? Day-to-day responsibilities include scheduling members and conducting home visits with members, as well as the daily operations that Siu Cheng's team completes
Any required languages they need to speak? Any language is accepted, but Bengali, Russian, Chinese or Spanish are preferred
The job description mentions 50-80% travel across all 5 boroughs. Could you please confirm if this travel requirement is accurate?
o We typically assign by zip code and we try to keep travel to a minimum as much as possible but we may need travel to other boroughs other than their home one as needed.
That being said, candidates that live in Long Island or Westchester county are highly desirable.
Candidates are primarily asking how many days per week they will be required to travel. Could you provide clarity on this?
o Visits are done 4 days a week with one day remote to schedule visits for the week.
Are candidates allowed to use public transportation or cab / shared services for travel, similar to the UAS Assessor role?
o Public transportation is allowed, cabs / shared services may not be used as a primary form of transportation but it can be used to supplement public transportation, especially in places where there is no easy way to travel from a station (i.
e. Long Island and Westchester, as well as some parts of Brooklyn)
Could you specify if the member visits will be conducted at their homes or in community / skilled nursing facilities?
o Homes, Community / Skilled Nursing facilities (if members reside), Adult Living Facilities, and they may also conduct visits as Social Day Care
How many member visits are expected to be completed per day?
o 5 / day to make the weekly minimum of 20 cases and the monthly expectation of 85 cases.
Will they be going on these visits alone to member's homes or with another person?
o All visits will be conducted alone, however, in areas where safety is concerned we offer escort services.
Is each contractor expected to cover all five boroughs, West Chester, and Long Island (that's a lot of area) or will they be divided up based on location of the contractor?
o Please see the answer to the first question. We will do our best to provide assignment closer to candidates borough of residence, but travel to other areas may be needed.
However, we do not expect candidates to travel from, for example, Bronx to Suffolk County in one day.

Position Overview :
Community Connectors works with members, providers and multidisciplinary team members to assess, facilitate, plan and coordinate an integrated delivery of care across the continuum, including behavioral health and long term care, for members with high need potential.
Staff work to ensure that patients progress toward desired outcomes with quality care that is medically appropriate and cost-effective based on the severity of illness and the site of service.
Essential Position Responsibilities :
Serves as a community based member advocate and resource, using knowledge of the community and resources available to engage and assist vulnerable members in managing their healthcare needs.
Collaborates with and supports the Healthcare Services team by providing non-clinical paraprofessional duties in the field, to include meeting with members in their homes, nursing homes, shelters, or doctor's offices, etc.
Empowers members by helping them navigate and maximize their health plan benefits. Assistance may include : scheduling appointments with providers;
arranging transportation for healthcare visits; getting prescriptions filled; and following up with members on missed appointments.
Assists members in accessing social services such as community-based resources for housing, food, employment, etc.
Provides outreach to locate and / or provide support for disconnected members with special needs.
Conducts research with available data to locate members the company has been unable to contact (e.g., reviewing internal databases, contacting member providers or caregivers, or travel to last known address or community resource locations such as homeless shelters, etc.)
Participates in ongoing or project-based activities that may require extensive member outreach (telephonically and / or face-to-face).
Guides members to maintain Medicaid eligibility and with other financial resources as appropriate.
50-80% local travel may be required. Reliable transportation required.
Position Qualifications :
HS Diploma / GED
Minimum 1 year experience working with underserved or special needs populations, with varied health, economic and educational circumstances.
Must have valid driver's license with good driving record and be able to drive within applicable state or locality with reliable transportation.
