- Develop preliminary and comprehensive assessments of the recreation service needs of each resident/patient.
- Encourage the resident/patient/family to participate in the development and review of his/her plan of care.
- Ensure that all recreation services personnel are aware of the care plan and that care plans are used in providing daily
- recreation services to the resident/patient.
- Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.
- Facilitate group programs as assigned in a timely and efficient manner
- Complete required documentation.
- Contribute to resident care planning preparation
- Attend and contribute to Activities Services Department meetings.
- Provide individual programming as assigned
- Maintain activity department supplies and equipment.
- Work with volunteers as assigned by Activities Services Director/Coordinator.
- Assist with planning and leadership special events.
- Adhere to universal precautions and sanitary, safety and infection control policies and procedures.

MINIMUM SKILLS /EXPERIENCE/EDUCATION
- Bachelor's (preferred)
- Supervising Experience: 1 year (Preferred)
- Social Work: 1 year (Preferred)
Salary : $38,100 - $48,300
