What are the responsibilities and job description for the Care Manager position at ArchCare?
Overview
The Care Manager plans and coordinates care management for members. The Care Manager works in collaboration with Clinical Services Associate to collaborate on the plan of care for the member, and also monitor effectiveness of the care plan and promote member satisfaction with the service provision.
Responsibilities
- Communicates with enrollment personnel to ensure a smooth transition at the start of care.
- Educates members and family in health management and community resources.
- Develops treatment plan, evaluates member progress regularly, and modifies treatment plans when necessary.
- Provides direct services, such as counseling, psychotherapy, and case/care management.
- Maintains oversight for the member’s care management during long-term SNF placements and short-term sub-acute placement for rehabilitation.
Education Requirements
- Graduate of an accredited Nursing or Social Work Program.
Education Desired
- Bachelor's degree in Nursing OR Masters degree in Social Work
Experience Requirements
- Two (2) years of experience in a community based program caring for older adults.
- Demonstrated skills in planning, organizing and directing the work of others required.
License Requirements
- Registered Nurse OR Licensed Masters Social Worker w/ NYS registration
