Case Manager for Adults with Disabilities

Essential Duties and Responsibilities:

• Establish rapport to have an effective working relationship with individuals/families/guardians.
• Provide case management for individuals. Work closely with, and act as the liaison between collaborating agencies/multi-disciplinary team.
• Develop, monitor, and document mandated services and programming, and ensure client satisfaction. Ensure individual files are audit ready.
• Understand and participate in all relevant assessments to ensure appropriate level of care for individuals.
• Conduct home visits, monitor, and support host homes. Provide technical support.
• Participate in rotation for “on call” to support our individuals.
• Support individuals at Interdisciplinary team (IDT Team) meetings, and advocate as appropriate.
• In a timely manner, effectively and appropriately communicate potential risks and/or threats to an individual or program to Ariel supervisors and collaborating agencies.
• Participate in ongoing program evaluation to ensure program effectiveness.
