Job Description: Case Manager - Evening Shift (SUD Clinic)
We are seeking a dedicated and compassionate Case Manager to join our Substance Use Disorder (SUD) Clinic team. As a Case Manager, you will play a vital role in providing comprehensive support and assistance to individuals seeking treatment for substance use disorders in the community. This position is specifically for the evening shift, ensuring that our services are accessible to those who may require assistance outside of regular business hours.
Responsibilities:
- Conduct thorough assessments of clients' needs, including substance use history, mental health status, and social determinants of health.
- Collaborate with clients to develop individualized treatment plans that address their unique needs and goals.
- Coordinate and facilitate referrals to appropriate community resources, including detoxification services, counseling, medical care, and support groups.
- Monitor clients' progress throughout their treatment journey, providing ongoing support, advocacy, and crisis intervention as needed.
- Conduct regular check-ins and follow-ups with clients to ensure compliance with treatment plans and address any barriers to recovery.
- Maintain accurate and up-to-date documentation of client interactions, treatment plans, and progress notes in accordance with clinic policies and regulatory requirements.
- Collaborate with a multidisciplinary team, including physicians, therapists, and other healthcare professionals, to coordinate and optimize client care.
- Stay informed about current trends, research, and best practices in the field of substance use disorder treatment.
Requirements:
- Valid Ohio Medicaid ID is a must.
- Previous experience working in a substance use disorder treatment setting is highly desirable.
- Knowledge of Ohio Medicaid policies and procedures.
- Familiarity with community resources and referral networks for substance use disorder treatment.
- Strong communication and interpersonal skills to effectively engage and build rapport with clients from diverse backgrounds.
- Ability to work independently and as part of a team in a fast-paced and dynamic environment.
- Excellent organizational and time management skills to effectively prioritize tasks and meet deadlines.
- Proficiency in electronic health record systems and other relevant software applications.
