What are the responsibilities and job description for the Care Manager position at Arosa?
Dynamic experienced Aging Life Care Professional / GCM; a hybrid role :
2/3 Pure Care Management: Direct work with our clients and their family members; provide caregiver oversight for our own employee caregivers. Based on training and experience, conduct professional assessments, develop a care plan, manage the plan: involvement in the implementation of needed services, providing oversight and reassessments.
1/3 Professional Outreach: Be our face in the community: Participate on local committees, task forces, and boards. Tasks may include: education / presenting to professional groups; raising consciousness and helping professionals learn about how we can help them help their clients/patients; set up meetings with professionals, get to know them, figure out what they need/want and help create solutions.
It takes a special professional, who is as comfortable with older clients and their families, as they are with Trust Officers, Attorneys, Physicians, etc... We are looking for someone who has confidence in what they do. Who can convey, "I'm so glad you called us, as I know I can truly help you" and really mean it.

Supervisory Responsibilities
Essential Duties & Responsibilities:
- May supervise in-home staff, including safety monitoring, performance management, training and all other supervision.
- Educational Preferred Requirement: Master’s Degree in Social Work or related field.
- Acceptable candidate: Bachelor’s Degree in Social Work, Nursing, Gerontology, Psychology, Health and Human Services or other related fields. Licensed LVN or RN. Licensure in Social Work highly desired.
- A minimum of two (2) years’ experience in geriatrics and/or care management required; other professional experience may substitute.
- Attainment of a Care Management Certification (CCM or CMC) required within the first year of employment.
- Ability to interface effectively and professionally with client committees, including family, physicians, attorneys, etc.
- Ability to work in a team environment required.
- Excellent written and verbal communication skills required.
- PC literacy required.
- Valid driver’s license and proof of current auto insurance required.
