Now Hiring –* Patient Care Manager - North Region*
*
Hours: Monday - Friday 8:30am - 5:00pm*
The Patient Care Manager is responsible for the direct day-to-day supervision and management of the Interdisciplinary Team in its provision of hospice care. Job duties can include, but are not limited to:
- Directs the provision of hospice care through oversight of the IDT
- Supervises and manages RNs, LPNs, and on-call team
- Manages Paid Time Off (PTO) requests for all RNs, LPNs, and on-call team
- Completes performance evaluations for all RNs, LPNs, and on-call team
- Completes annual and PRN competencies/supervisions
- Provides mentoring/orienting for new hires
- Conducts case conferencing at least monthly
- Manages scheduling/liaison with intake for admissions
- Prepares end of day report(s)
- Conducts required audits
Qualifications
Education:
Associate's Degree in Nursing

Professional Experience: *
- Must have a State Certified RN or LPN license
- 2 years' experience as a professional nurse, hospice experience required
- Must have a valid Driver's license and reliable transportation
- Must be willing to travel
- Strong verbal and written communication skills.
