What are the responsibilities and job description for the Registered Nurse- RN Designee position at Ascentria Care Alliance?
Luther Ridge, part of non-profit Ascentria Care Alliance, is a beautiful independent and assisted living community located in Middletown, Connecticut. We are seeking a dedicated RN Designee to assist with managing resident care services that are provided in the community. The management part of the RN Designee will include supervision of CNA caregivers and LPN’s, resident assessments, and management, as defined by the Supervisor of Assisted Living Services (SALSA). The RN Designee acts as the SALSA Designee when the SALSA is not present. In addition to the essential responsibilities that follow, having a passion for enriching the lives of older adults is amongst the most important requirements in this position.
Essential Responsibilities:
- Manages and coordinates the work of assisted living staff CNA’s and LPN’s.
- Assesses the physical and emotional needs of residents. Makes referrals when appropriate to other community agencies and resources.
- Maintains a positive and professional demeanor towards employees, residents and families.
- Assesses potential residents for appropriate level of care as directed by the SALSA.
- Writes, updates and reviews resident assessments and care plans with appropriate staff
- Strives to maintain a safe work environment through the prevention of accidents and implementation of safe working practices.
- Makes referrals to appropriate professionals or agencies, whenever the client’s condition necessitates, including the provision of current clinical information ensuring that if the client’s condition is no longer chronic and stable, services of a licensed home health agency are engaged or other appropriate arrangements are made.
- Adheres to all company policies and procedures.
- On call responsibilities shared with SALSA
Personnel Specifications:
- Current and Valid CT RN license
- Either (A) a baccalaureate degree in nursing and a minimum of two (2) years full-time or full-time equivalent clinical experience in nursing, at least one (1) of which shall be in a home health care agency or community health program that included care of sick at home, or (B) A diploma or associates degree in nursing and at least four (4) years full-time or full-time equivalent clinical experience in nursing, with at least one (1) year of which shall be in a home health care agency or community health program that included care of sick at home.
- Computer skills including use of word processing and spreadsheets
- Knowledge and compliance with all Connecticut Assisted Living Services Agency regulations.
