We are a top-tier private duty Home Care Agency based in Norwich, CT, and we provide our care professionals with the right tools and resources to help our clients receive the best care experience. We are looking for healthcare industry professionals (Medical Assistants, Certified Nursing Assistants, Scribes, Patient Care Technicians, Phlebotomists, and related) to join our team and help us preserve the dignity of our senior citizens living at home.
This is a non-medical role that requires experience working within a healthcare system. Care Coordinators will be expected to understand the healthcare professionals you will interact with while assisting in coordinating your client’s care. You will work under the supervision of the Case Manager. If you don’t have prior hands-on care experience and training, once hired, you will attend a Personal Care Aide training program as part of your orientation, followed by continuous support from the case manager.

As A Care Coordinator, You Will Be:
- Providing personal care and companionship to the client.
- Managing the client’s home and appointments
- Planning meals, purchasing food, and preparing it for the care recipient.
- Taking the client out for socialization
- Performing light housekeeping and laundry to maintain clean and hygienic living space.
- Driving the client to their appointments within the local area.
- Assisting the client in and out of the vehicle.
- Communicating the client’s challenges and problem-solving with the agency and care team.
- Collaborating with other care professionals to ensure continuity of care
Requirements Experience/Training:
- Strong verbal and written communication skills
- Competent computer skills (Google Suite)
- An associate degree, healthcare certificate, and/or 2 years of experience in a coordinator role
- Problem Solver
- Demonstrates consistent career growth pattern
- Commitment to ongoing training
- Valid driver’s license and an insured vehicle
