Aston Carter is looking for a Clinical Care Coordinator to join a large healthcare organization to cover a maternity leave for 4 months. **CVD 19 vaccination required **

Description:
The Clinical Care Coordinator is responsible for the overall organization of resident medical records, supplies, DME and administrative duties as outlined on the assigned unit(s), which includes scheduling medical appointments/ transportation, keeping track of labs and diagnostic reports, maintain needed resident medical supply par levels and assisting with timekeeping and attendance for staff on assigned unit(s).
Description:
- Schedules resident medical appointments and transportation for those appointments.
- Ensures prior approval for transportation payment is secured. Ensures on-line appointment calendar is up to date
- Compiles required information for medical appointments and assists with obtaining proper releases, consents.
- Responds to phone calls and addresses the needs of the caller appropriately.
- Assists with timekeeping and attendance for staff on assigned unit(s).
- Prepares new medical records for all new admissions/re-admissions and completes all required labeling and closing out of discharged resident records.
- Tracks all labs and diagnostic reports for residents on assigned unit
- Ensures Red Star designations are in place.
- Maintains up to date documents/ worksheets and forms used on the Nursing Units as needed.
- Files/stores nursing department documentation.
Pay Rate: $19/hr
Hours: M-F 7:00am-3:00pm OR
M-F 8:00am-4:00pm (37.5 hours/week)
