Full Job Description

Position Summary: Responsible for facilitating all aspects of the intake process. Collects and reports statistical data relating to referral sources. Monitors patient census and compiles weekly census reports. Act as a liaison for patients and families, as well as coordinating third-party vendors.

Essential Functions:

  • Responsible and accountable for the referral intake and reporting process, including communicating with agency staff to ensure paraprofessional availability for new cases.
  • Entering referral information and maintaining documentation in EMR.
  • Obtains service authorizations.
  • Assists with vendor inquiries related to intake coordination.
  • Maintains working knowledge of home care rules and regulations/Medicare, Medicaid and other third party payer.
  • Educates potential referral sources about home care.
  • Explains insurance coverage to prospective patients and referral sources.
  • Notifies appropriate staff of pending referrals.
  • Establishes and maintains positive relationships with customers and referral sources.
  • Responds to customer requests and concerns.
  • Monitors patients who are hospitalized or out of service area, and contacts patients/families as needed for collaboration of care.
  • Completes weekly statistical report pertaining to intake and discharges.
  • Complies with all agency policies and procedures.
  • Adheres to HIPPA guidelines.
  • Provides patients with benefits and entitlements counseling as needed.
  • Provides patients and families with emotional support and assistance with psychosocial issues as needed.
  • Participates in cases conferences and acts as resources to other team members in the identification and resolution of patient issues.
  • Maintains and submits documentation in accordance with agency policy.
  • Track referrals that are submitted.

Non-Essential Functions: * Attends all required training, in-service, and staff meetings.

  • Maintains a positive and professional demeanor toward clients, visitors, families, and co-workers.
  • Adheres to all policies and procedures.
  • Performs other duties as assigned.

Qualifications/Skills/Educational Requirements:

Demonstrated psychosocial support skills for patients and families.

Basic competence with computers, including Microsoft Word and Excel.

Fluent in Spanish required
