Job: Registered Nurse - RN
Atrinity Home Health is a Medicare Certified, State Licensed Home Health Agency servicing all of Connecticut. We are looking for a Baylor Registered Nurse for a position in home health. The registered nurse must have at least 1 year of experience with adult/geriatric patients in the hospital, skilled nursing facility (SNF), outpatient or home health setting. Pay and benefits are very generous.

Requirements - Registered Nurse - RN
· Current State of Connecticut Registered Nurse License
· At least one (1) year of clinical experience
· Current driver's license
· Reliable transportation
· Strong clinical assessment, organizational, and communication skills
· Ability to work independently and as part of a team.
· Provides skilled hands-on care to patients in the comfort of their own homes
Responsibilities - Registered Nurse - RN
· Develop the patient's plan of care for the safe and effective delivery of services in their homes
· Work closely with an interdisciplinary team to achieve optimal patient outcomes
· Provide education to empower patients to manage their chronic disease processes
